Have you ever noticed that your body seems to hurt more when life gets stressful?
Maybe your back starts bothering you during a hectic week at work. Your old knee pain flares up when you’re not sleeping well. Or an injury that had been feeling pretty good suddenly seems more noticeable when you’re overwhelmed.
You’re not imagining it.
Stress can change how your nervous system processes pain.
That doesn’t mean your pain is “all in your head.” Pain is real, and stress doesn’t mean an injury or medical condition isn’t real. Instead, stress is one of many factors that can influence how strongly your brain and nervous system respond to signals coming from your body.
Pain Is More Than a Signal From Your Body
It’s easy to think of pain like an alarm: tissue gets injured, a signal travels to the brain, and you feel pain.
The reality is more complicated.
Your nervous system is constantly taking in information from your body and deciding how much of a threat that information represents. Your brain then helps determine how strongly you experience that threat as pain.
This is one reason two people can have similar injuries but experience very different levels of pain.
It also helps explain why your pain can change from one day to the next—even when the underlying tissues haven’t changed very much.
So What Does Stress Have to Do With It?
When you’re stressed, your body shifts into a state designed to help you deal with a threat. Your heart rate, muscle tension, attention, and other body systems can change.
That response is useful in short bursts. But when stress becomes persistent, it can affect how your nervous system processes sensory information.
Research has found that psychological stress can increase pain sensitivity in some people, while reducing the body’s ability to dampen pain signals.
Think of it like turning up the volume knob on your nervous system.
The painful stimulus may not have changed—but the way your system responds to it can.
Stress, Sleep, and Pain Can Become a Cycle
Stress rarely happens by itself.
A stressful week may lead to poor sleep. Poor sleep can make you more sensitive to pain and less able to cope with it. Pain can make it harder to sleep or exercise. And reduced activity can sometimes make your body feel even worse.
You can end up with a cycle:
Stress → Poor sleep → Increased pain sensitivity → Less activity → More stress
The good news is that you don’t necessarily have to fix everything at once to start breaking the cycle.
What Can You Do?
You can’t eliminate stress from your life—and you don’t need to.
Instead, focus on the things you can influence.
Keep moving.
When appropriate, continue your normal activities rather than completely avoiding movement because you’re having a painful day.
Prioritize sleep.
A good night’s sleep won’t magically eliminate chronic pain, but consistently poor sleep can make pain harder to manage.
Use stress-management strategies.
Walking, breathing exercises, meditation, time outside, hobbies, and connecting with other people can all provide ways to help your nervous system settle.
Don’t catastrophize a flare-up.
A bad pain day doesn’t necessarily mean you’ve caused new damage. Sometimes your symptoms are responding to the overall stress on your system.
And, of course, new or significantly changing symptoms should be evaluated by an appropriate healthcare professional.
The Bottom Line
Pain is real. Stress is real. And the two can influence each other.
Your pain isn’t “just in your head.” But your brain and nervous system are involved in every pain experience, and things like stress, sleep, activity, and your environment can influence how strongly pain is felt.
So the next time you have a particularly painful week, take a broader look.

The answer may not explain everything—but it may give you another piece of the puzzle.
